The Opportunity: Registered Nurse, Multispecialty - Mass General Brigham Integrated Care (Salem, NH)


Why This Role Is Important


We are looking for an enthusiastic and passionate Nurse to partner with our clinical and non-clinical primary care and multispecialty staff to test out new ways of delivering exceptional care to our patients. This role is a unique opportunity to work on-site with the clinical team to deliver high-quality patient care, as well as remotely to support clinical coordination activities. The Nurse is an integral part of our diverse team, embracing our culture of well-being and kindness, and contributing to constant improvement. This role will be part of a team that will frequently prototype new ideas, including digital tools, care team roles, and workflows, that may be used in designing for the future of ambulatory care across Integrated Care. What You'll Work On


The RN will be primarily accountable for:


  • Working closely and collegially with physicians, APPs and other clinical and administrative staff to enhance the delivery of patient care services along the continuum of care

  • Being the liaison between the patient, surgeon, and PCP when needed to assist in preparing the patient for any surgeries or procedures including but not limited to gathering documentation and written plans for anticoagulation or medication.

  • Ensuring that patients receive timely, high quality and efficient health care and support services

  • Promoting patient satisfaction by interacting with patients in a professional, friendly manner consistent with their needs


Clinical Patient Care Responsibilities:



  • Evaluation of patients in clinic, as determined by an MD

  • Triage and evaluation (on site or virtually)

  • Assisting the care team to remain in flow

  • Administering injections, medications and providing complex wound care (on site)

  • Assisting with minor surgery, office laboratory and other tests (on site)

  • Instructing patients regarding preparation for various tests and procedures

  • Medication teaching (e.g. insulin starts, lovenox injections, etc.)

  • Assisting care team when office emergencies arise

  • Providing hand off to EMS and ED (in collaboration with provider)

  • Acts as a resource to other clinical staff in the practice

  • Monitoring patient(s) and providing support to patient/family

  • Assists patients and families with self-management of chronic diseases, including but not limited to: Hypertension, Diabetes, Hyperlipidemia, Obesity, CHF, COPD, Asthma, and Depression

  • Assesses patient/family continuing care needs in collaboration with the care team

  • Coordinates care transitions across the health care delivery system including visits to specialists, visits to the Emergency Department or Urgent Care, admissions to inpatient and extended care facilities; and transitions to different models of care, such as long-term care or hospice.

Clinical Coordination Responsibilities Include:


  • One call resolve

  • Complex triage/addressing clinical questions

  • Refill set-up

  • Symptom screening and routing

  • After visit/post procedure check-in

  • New medication/symptoms check-in

  • ED/Inpatient follow-up calls

  • VNA coordination

  • Obtaining authorization for services/equipment






Qualifications

About You



  • Completion of an accredited Nursing degree program required

  • Actively licensed as a Registered Nurse in the state of New Hampshire

  • BLS certification required

  • Proficiency with an EHR, Epic experience preferred
