Responsible for consulting with providers to improve the effectiveness and efficiencies of provider practices and clinical processes.

• Hours: 8-5pm in your time zone


• RN, LCSW, LMSW, LMHC, LPC, LBA


• W2 ongoing contract (conversions highly likely based on performance)


• 
  Must have behavioral health and psychiatric experience
  


• 
  Behavioral Health experience is a must
  


• 
  Clinical quality experience as well as experience speaking with providers
  

The Care Consultant will have a list of providers identified for the clinical initiative


You will make phone calls to the providers and have conversations about best practices, etc. Making call-outs to schedule providers or have those provider meetings. The expectation is 4 actual provider meetings per week (approx. 45 min)



Responsibilities:

• Analyze practice-specific data to pinpoint cost and quality improvement opportunities.


• Assist practices in targeting high-risk members and addressing identified care gaps.


• Support providers in implementing population health management, care coordination, and care management strategies.


• Develop and implement action plans to improve cost, quality, and patient experience.


• Facilitate program onboarding and routine meetings between the company and provider offices.


• Serve as the subject matter expert and liaison with Care Management to streamline clinical processes.


• Design and deliver tailored learning opportunities for providers on program interventions.


• Host collaborative learning events fostering peer-to-peer and national expert knowledge sharing.


• Utilize Provider Care Management Solutions (PCMS) and other data sources for program administration.


• Provide technical support for the CDT program within the Enhanced Personal Health Care (EPHC) program.


• Collaborate with the EPHC team and stakeholders (CDT Market Lead, EPHC Provider Solutions team) to meet provider needs.

Requirements:

• Current active unrestricted licenses such as RN, LCSW, LMSW, LMHC, LPC, LBA (as allowed by applicable state laws), LMFT, or Clinical Psychologist to practice as a health professional within the scope of licensure in applicable states or territory of the United States required.


• Licensure is a requirement for this position.


• BA/BS;


• 5-10 years of health care experience; or any combination of education and experience, which would provide an equivalent background.


• Physician environment experience in practice transformation/quality improvement, ambulatory care setting quality and efficiency metrics, population health and care management structures, data analytics, and electronic health records preferred.


• Clinical background and/or MBA not required but a plus.
