Community Case Manager

A Case Manager with Compass provides goal directed services to children with mental health needs. This service may be with the child in the school, the home, and/or the community. Our goal is to help children in the environment in which they need it the most.

GENERAL RESPONSIBILITIES:        A certified case manager works to provide quality case management services in an outpatient or community setting designated by CBH. The primary function of the adult case manger is to provide goal directed supports and solution focused interventions intended to achieve identified goals or objectives as set forth in the consumer’s individualized treatment plan. CPST (Community Psychiatric Support and Treatment) is a face to face intervention with the consumer present: however, family or other collaterals may also be involved.  These services are provided in both agency and community settings, such as the client’s residence.  The case manager is responsible administratively to the Program Director. 

 MINIMUM QUALIFICATIONS:  A BA/BS degree with an emphasis on Mental Health/human services or equivalent experience. Staff members shall possess demonstrated interpersonal skills, ability to work with the mentally ill, ability to respond effectively in a wide variety of human service situations. Experience working with persons diagnosed with a Severe and Persistent Mental Illness is preferred.  Must have a current and valid Kansas driver’s license.  Must successfully complete Case Management Certification as stipulated by the state of Kansas within six months of employment and maintain Case Management  Certification there after.

CERTIFICATES, LICENSES, REGISTRATIONS: Certification as Case manager through on line training of training teams.org and Basic Case management from University of Kansas - to be completed within 6 months of employment.
