Purpose Statement / Position Summary
Under the direction of the Executive Director of Accountable Care, the Case Manager for the ACO Program will follow a collaborative process to assess, plan, implement, coordinate, monitor and evaluate the options and services required to meet an individual's health needs. The Case Manager for the ACO program is responsible for the identification, evaluation and coordinator of all applicable health care services provided to the attributed ACO population and patients identified as high risk with two or more chronic health conditions.

To perform this job successfully, an individual must be able to perform each essential duty listed above with:

  • Strong verbal and written communication skills

  • Ability to complete work independently and collaborate with internal and external team members and participate in ACO and/or related meetings, committees or functions

  • Computer literacy with ability to work within several internal and external on line and Web based systems, reports and analytics

  • Basic Math Skills

  • Understanding of privacy and HIPAA standards

  • Flexibility and multi-tasking skills 

Minimum Requirements / Work Experience

  • Three years in acute care setting

  • Previous ACO Program preferred

  • Understanding of privacy and HIPAA standards

  • Proficient computer use (includes typing, Internet, MS Office, Analytics Software)

  • Able to work independently

  • Bilingual Spanish a plus


Education / Licensure / Certification

  • Current California  RN license in good standing required

  • 3 years of case management experience and or CCM (preferred)
