The MSHO Care Coordinator coordinates the provision of Medicare and Medicaid community and long-term care services for MSHO enrollees, and coordinates services to an MSHO enrollee among different health and social service professional and across settings of care, in accordance with State, Federal and health plan requirements. Works with defined populations seeking care for health promotion, health maintenance, or health related problems. The role of the MSHO Care Coordinator is to skillfully assess the patient’s health and response to interventions, and offer education and counseling that lead to improvement of the patient/family’s self management skills. Develops enrollee care plans and evaluates the effectiveness of care in the context of the patient/family’s goals and lifestyle and takes necessary action to promote optimum outcomes. Nursing actions are based on principles of respect, advocacy, research and evidence-based practice, interdisciplinary collaboration, clinical coordination/care management, leadership, and outstanding communication and customer service. Nursing practice is relationship based and can occur in single or ongoing encounters, in person (office or patient home), telephonically or using computer-based communication. Position requires high level of customer service skills to establish and enhance positive relationships with patients, co-workers, and others.

QUALIFICATIONS (minimum requirements) Education and/or years experience required: •Graduate of an accredited school of nursing. Licensure/Certifications required: •Current registration or permit to practice professional nursing in appropriate state. •Must have reliable transportation to enrollees’ residences and valid driver’s license.
