Job Description:

The Patient Care Coordinator, in a professional and courteous manner, coordinates respiratory therapy services and provides delivery and maintenance services in patient homes or office setting. Customer files are electronic and must be maintained, with information to include but not limited to: orders, reimbursement forms, and plan(s) of service.

Primary functions include, but are not limited to:

* Handling delivery and set-up of equipment in patient homes and office settings

* Taking Referrals

* Knowing reimbursement requirements for each modality

* Promptly giving referral to appropriate staff member

* Ensuring correct training/education of patients and/or caregivers in the safe and proper use of equipment purchased or rented

* Being available to perform on-call functions as required by the needs of the location on a twenty-four hour basis

* Having personal transportation, a telephone, and an off-hours schedule permitting rapid completion of any emergency on-call duties that may arise

* Ordering equipment and supplies for clinical programs

* Preparing rental agreement, invoice, admission, and insurance verification forms

* Ordering and pulling supplies and check inventory

* Testing functionality on certain respiratory equipment

* Consulting with and coordinating patient treatment, medical conditions, and home environment for Respiratory Services

* Assisting with paperwork required for high volume therapies

* Assisting in the assessment of the home environment of patients receiving equipment and/or services from AHP to ensure patient safety

* Patient selection or calling clinician if necessary (i.e. set up or new patient)

* Extensive understanding of all aspects of treatment modalities offered by branches

* One to two years' experience in healthcare industry

* Understand medical terminology

* Valid driver's license, automobile insurance, good driving record for the past three years

* Must be 18 years or older to apply

* Must have High School Diploma/GED
