Location: Cincinnati, Ohio
Primary Purpose:To support the development of patient centered, team-based care; facilitate high value care that improves timely access to and provision of preventive and chronic disease treatment. The Care Plan Coordinator uses critical thinking skills to identify patient's care gaps, engage patients in their own care, gather information for treatment interventions, and enhance communication between the patient and his/her care team.
Knowledge and Skills:
Education: Associate's degree in healthcare or related field, preferred.
License and Certifications: Medical assistant certificate
Years of Experience: Three to five years equivalent experience, minimum required.
Required Skills and Knowledge: Prefer experience in a busy primary care setting. Proficient computer skills including Microsoft Office suite. Prefer experience with electronic health record.
Ability to work without close supervision and set own priorities. Ability to manage multiple tasks and successfully meet deadlines.
