Job Description:
NURSE CLINICIAN - INPATIENT CASE MANAGEMENT

Position available include:
  • 100%, Days, Monday - Friday, 8 hour shifts between the hours of 7:00AM and 7:00PM as well as weekend and holiday rotations.
  • Function as a clinical nurse expert within the Medical Surgical Services Units and care coordinator for the assigned patient population facilitating seamless transitions across the continuum of care.
  • This position may also see some psychiatric nursing patients
  • The nurse functions as an administrative and clinical liaison for individuals to optimize quality health care delivery using clinical assessment/screening tools, education reinforcement, follow-up on the plan of care recommendations and guiding the utilization of cost-effective resources.
  • This position will manage a specified population and work closely with other care coordinators during transitions of care as well as other interdisciplinary team members across the Continuum of Care.

Required Education/Experience includes: • A Baccalaureate degree in Nursing is required. • Current license to practice nursing in Iowa is required by date of hire. Desirable Qualifications includes: • Two years recent adult medical/surgical inpatient experience, including care coordination is required. • Proficiency in computer applications including Microsoft Excel, Word and PowerPoint is required. • Excellent written and verbal communication skills are required. • Demonstrated excellent organizational skills, critical thinking, and problem solving skills are required. • Demonstrated expertise with adult patient/family discharge planning process is required. • Experience working with an interdisciplinary team is required. • Previous case management experience is desired. • Certification in case management is desired. • Previous experience with EPIC is desired.
