The Diabetes Coord has the knowledge in regulatory and quality certification requirements r/t glucose outcomes. As the subject matter expert, the will identify the complex inpatient who requires diabetes education and support; and provides the education personally and/or guides the staff nurse on required learning needs prior to discharge. Collaborates with the patient care team members to provide updates on glycemic matrix for SMC and current trends in diabetes care.
3+ Years of Diabetes Education in an inpatient setting preferred
3+ Years of working as a Diabetes Educator in an ADA or DEAP outpatient setting preferred
1+ Year as a Program Coordinator for either an ADA or DEAP program preferred
Extensive knowledge in smart insulin pens and diabetes injectables preferred
Knowledgeable in both subcutaneous glucose sensors and continuous glucose sensors
Bachelor's Degree or higher in Nursing preferred
Current California Registered Nurse License required
Active Certified Diabetes Educator (CDE) preferred
Board Certified - Advanced Diabetes Management (BC-ADM) preferred
