JOB SUMMARY:

The RN Case Manager ensures appropriate utilization of resources, facilities, and services by the review of admission, continued stay, and professional services rendered. Identifies patient discharge needs and works in collaboration with nurses and physicians in managing the patients care needs along the continuum. Establishes and maintains a good rapport and professional relationship with fellow employees, physicians, patients, and other departments. Follows policies and procedures of West Florida Hospital. Participates in the Quality Improvement process and adheres to WFH standards of behavior.

QUALIFICATIONS:

* Associate's Degree (or Diploma) in Nursing is REQUIRED; Bachelor's Degree in Nursing (BSN) is preferred.

* Florida Registered Nurse (RN) license or approved multi-state license REQUIRED

* A minimum of one (1) to three (3) years of nursing experience REQUIRED

* Case management experience preferred
