Job Opportunity:

We are looking for an organized, detail oriented, analytic Aftercare and Discharge Specialist to improve program effectiveness, resident outcomes and contractual requirements in San Diego County.

The Aftercare and Discharge Specialist is responsible for data collection and data entry for all assigned programs, in addition to, the analysis of all data related to program effectiveness, resident outcomes, and contractual requirements. The Aftercare and Discharge Specialists is also responsible for maintaining accurate, up-to-date and usable information in the agency's various electronic databases. This position will be primarily in San Diego and the National City location.

Job Duties:

  • Coordinates level of care assessments, treatment plans, determination of diagnosis, discharge plans, case management and clinical notes with the interdisciplinary team to ensure complete and in compliance.

  • Meets documentation requirements by auditing social history, assessments, progress notes, as well as, ensuring completion of some aspects of admission paperwork, and ensuring ASAM (American Society of Addiction Medicine) requirements are met.

  • Participates weekly in the interdisciplinary team meetings as a administrative support .

  • Establishes, maintains, and closes client charts in an orderly manner by completing regular and ongoing data quality review of completed work.

  • Identifies any issues associated with data quality and data integrity and coordinate with supervisor to develop a plan to address concerns.

  • Develops and maintains a working knowledge of all agency and partner database systems including Welligent (Electronic Health Records), Office 365, and SanWITS (Web Infrastructure for Treatment Services).

Job Qualifications:

  • Must have an Associate Degree in Psychology, Sociology, Social Work, Business, or Accounting or 60 hours of college coursework leading towards a Bachelor's Degree in related area.

  • Must have data entry experience and/or Medical and medical billing experience.

  • Must have a valid California Driver's License.

  • Excels at working with and meeting deadlines

  • Consistently performs assigned tasks promptly and accurately

  • Detail oriented

  • Can quickly learn how to use an online database management system

  • Must obtain CPR and First Aid certification within 3 months of employment.

  • Has good information collection and management skills

  • Ability to work under pressure

  • Must be able to lift and carry up to 25 lbs.

  • Some travel required.
