Provides evaluation, reassessment, and treatment of patients referred for home health physical therapy and supervises the plan of care and clinical work of the PTA's assisting with the care. Serves as the home health case manager on therapy only cases taking responsibility for leading the care team, managing the total care plan and assuring adherance to the regulatory and agency guidelines for these cases.

It is expected that all duties and responsibilities of this position will be performed in a manner consistent with Our Mission, Our Core Values and The Mercy Standards.

Essential Functions (7-10 statements)

Completes accurate evaluations, reassessments and treatments of physical therapy patients. Oversees PTA treatments assuring adherance to plan of care and

quality outcomes. Develops patient plan of care and adapts the plan according to asssessment data and the patient needs. Schedules work independently 

and according to patient's clinical need and agency policy. Documents care, progress, goals, contacts, and orders according to policy and regualtions.

Serves as care team member, communicating effectively with patients, families, caregivers, physicians, and third party payors. Works autonomously without

direct supervision in the field. Uses computerized technology for clinical care effectively after on the job training. Serves as primary case manager on

therapy only cases. At discharge assures coordination with any needed ongoing services and completion of discharge teaching, Complies with and is 

knowledgable of hospital and department safety procedures and policies,

Department Specific Functions (2-4 statements)

Considerable driving in any weather and road conditions.  Complies with and is knowledgeable in reassessments frequencies to meet state and payor 

guidelines.  Is willing to assume additional responsibilities.

Minimum Required Qualifications

Education: Graduate of an accredited PT program    

Licensure:Kentucky licensed and Illinois license eligible 

Certification(s): 

Experience:     

Preferred Qualifications

Education: Clinical Doctorate in Physical Therapy from accredited PT program    

Licensure:Kentucky and Illinois licensed 

Certification(s): 

Experience: 1 year experience in home health setting
