Provides personal care services under the direction of a licensed nurse within the guidelines of the interdisciplinary team care plan to Homecare /Hospice patients.

It is expected that all duties and responsibilities of this position will be performed in a manner consistent with Our Mission, Our Core Values and The Mercy Standards.
Essential Functions (7-10 statements)
Assist patient with bath, mouth skin and hair care, using the bathroom/bedpan/commode, transfers,/ambulation, prescribed exercises. Perform incidental
household services that are essential to patients well-being at home. Takes patient vital signs, and report to RN as per POC. Assist with eating, preparing
meals- including special diet. Assists with shopping if patient has no other resources available. Accompanies patients to appointments (No driving permitted).
Reviews POC in home prior to providing service and documents according to agency standards. Provides emotional support and compassionate care to
patient/family. Practices active and empathetic listening- encouraging patient/family on life reflection. Reports significant changes in the patient's condition
or family situation immediately by calling the HP/HC office and notifying the PCC and/or the nurse case manager. Practices universal precautions according
to OSHA regulations and CHCI policy. Adheres to all HC/HP policies and procedural guidelines.
Department Specific Functions (2-4 statements)
Knowledgeable/complies with hospital/department policies/procedures. Contributes to care of environment by maintaining work area/assisting with light house-
keeping as needed. Knowledgeable/participates in hospitals performance improvement program. Utilizes computer system as needed for information and to
complete tasks. Maintains customer rights. Interacts with customers in a way that exemplifies hospital Core Values/ Mission. Attends annual in-service,
department meetings, and job related training sessions. Knowledgeable/adheres to hospital/department policies/procedures.

Minimum Required Qualifications
Education: High school graduate or GED
Licensure:Completion of Nursing assistant course or equivalent, driver's license with appropriate insurance.
Certification(s):BLS validation required.
Experience: Hospital/skilled nursing facility with acceptable references .
