Full-Time RN Case Manager – The Alton Memory Care

The RN Case Manager at The Alton Memory Care is responsible for the delivery of professional nursing care. The RN Case Manager will effectively direct the clinical staff to ensure the delivery of high quality resident care and services consistent with regulations and established best practices. 

Specifically, the RN Case Manager will:

 


  • Work full time, 40 hours, Monday-Friday, regular business hours + on call every 5-6 weeks.

  • Manage a caseload of 30-33 residents.

  • Participate in the planning and implementation of strategies and care delivery that assure high quality resident care, while achieving the short and long-term operational goals of the Community.

  • Establish and maintain effective communication with staff, residents and their families, physicians and other home care agencies.

  • Direct resident care processes: assessment, planning, implementation and evaluation of nursing care based upon regulations and established clinical best practices.

  • Training and tracking competencies of licensed and unlicensed staff.

  • Maintains and assures compliance with state regulations.

The RN Case Manager reports to the Homecare Director.

Special Requirements


  • Must be licensed as a Registered Nurse in MN and must maintain a current, unencumbered license with the MN Board of Nursing.

  • At least 3 years’ experience in areas such as nursing supervision or geriatric nursing preferred.

  • Working knowledge of state requirements for the Comprehensive Home Care License, etc.

  • Excellent judgement and supervisory skills.
