The Manager of Community Health is responsible for developing and managing programs which support Community Health within an assigned geographic area; focuses to include health equity, economic and community development, coalition building, and community engagement. Will create policies, procedures, goals and objectives for these programs.  Will manage Program Directors, Program Managers, and special organizational interests at the national and local levels.

Essential Functions:

  • Develop a working knowledge of existing conditions, trends, activities and public policies affecting the community, and assists with assessment of health status and conditions within the social environment

  • Provide significant assistance in creation of Community Health Needs Assessments within assigned geographic area

  • Interact effectively with local system management, elected officials and their staff, and community agencies, and potential funders or investors.

  • Develop effective relationships with community leaders and residents across diverse interests, cultures and socio-economic backgrounds, including formation of community coalitions and priorities

  • Assist in legislative and public policy advocacy to local and state government officials for issues that impact the health and well-being of the community

  • Implement priorities resulting from Community Health Needs Assessments

  • Provide input to proposals and capital requests related to community revitalization and/or economic development programs

  • Collaborate with and maintain network of civic partners in the service of addressing the health and wellbeing needs of communities

  • Engage state and county partners with attention to grant and funding opportunities; assist in preparation of grant applications

  • Serve as resource to local and market leadership teams in the promotion of effective community partnerships across the care continuum

  • Prepare internal and external documents, including evaluation and progress reports, board and public presentations, and funding requests


Minimum: Bachelors Degree, Public health, health services management, public administration, social services.


3 years progressive, related experience.

Supervisory experience desired
