The Program Manager for Population Health - Quality is responsible for successfully achieving multi-year business/functional outcomes associated with Blue Shield of California and Blue Shield Promise Health Plans Population Health Management Program. The ideal candidate will ensure completion of the Population Health Program or project is on schedule and within budget constraints. The Program Manager will be responsible for managing the ongoing activities associated with implementing and maintaining the Population Health Program to ensure ongoing alignment with Blue Shield of Californias strategy and company objectives. The Program Manager will support the day to day operations of the Population Health Program. The ideal candidate will lead cross-functional meetings and provide thought leadership that guides a team of population health professionals and influences/executes on the tactics and strategies that are undertaken to positively impact population-based care measures and factors that influence health. This position will report to the Director of Population Health Management. The Program Manager will work in partnership with leaders inside the organization to ensure the successful implementation of the overall Population Health Program for Blue Shield of California and Blue Shield Promise Health Plan. Partners will include Innovation teams, Healthcare Quality & Affordability, Customer Experience, Information Technology, Medical Care Solutions, Manifest Medex team, strategic provider groups, and Health Care Model of the Future pilot leaders. S/he will partner directly with internal partners and the Director of Population Health Management to ensure that the Population Health Program meets the needs of the population for Blue Shield of California and Blue Shield Promise Health Plan.

SPECIFIC RESPONSIBILITIES INCLUDE:

* Manage the population health management program across the enterprise, including overall compliance with accrediting bodies and strategic innovation.

* Manage day to day operations of complex innovation projects and the population-based care quality program. Responsible for managing project throughout the delivery lifecycle.

* Works closely within the Healthcare Quality and Affordability organization, across other business units and project teams and to ensure the Leadership Team is provided with information needed to make timely and effective decisions about innovative, population-based care and quality projects, priorities and activities.

* Develops and maintains an in-depth knowledge of program/project activities, progress, issues and risks. Constantly evaluates risks, opportunities, sequencing, scale, timing, and rationale for innovation projects and the program overall.

* Work with Director of Population Health to oversee financial management of projects and collaborates with BSC Information Technology to assist with project gating process for strategic funding.

* Develops comprehensive updates for senior management addressing potential opportunities, risks, contingencies, and/or course direction recommendations.

* Recommends implementation and scaling plans and strategies that will directly impact the achievement of overall program objectives.

* Responsible for proactively improving upon existing processes and systems using significant conceptualizing, reasoning and interpretation. Solutions are novel and unique and draw upon best practices based on expertise.

* Communicates with parties within and outside of own job function, and typically has responsibilities for communicating with parties external to the organization, which may include customers or vendors.

* Works to influence others to accept job functions view/practices, and agree to or accept new concepts, practices and approaches.

* Responsible for managing large, complex projects and overall program of strategic importance to the organization, involving large cross-functional teams.

* May direct the work of high-level individual contributors and/or act as a cross-functional team lead.
