SCHEDULE: 1.0 FTE (40 hours per week) SHIFT: Days with rotating weekends.
POSITION SUMMARY: The Case Manager Extender supports the Case Manager and Clinical Social Worker in coordinating/facilitating the arrangements for care/service/equipment required for patients to make a timely, seamless transition from acute care to the most appropriate level in the continuum capable of meeting the patients' needs.
Two years acute hospital experience or equivalent
One year working in a Case Management Department
High School Diploma or equivalent
Demonstrated communication, listening and skills.
Able to network with outside resources.
Must be available at all time when assigned to rotation.
Competency Checklist for New Staff (on hire)
Participates in goal setting within the department, assigned units and hospital. Assists manager and/or director with implementing department strategies for achieving goals.
Assists in data collection of patient information used to formulate the Case Manager's assessments, planning and interventions.
Works with patients/families, physicians and other members of the interdisciplinary team, and community referral sources in arranging for the care/service/equipment the patient will need upon discharge. This includes, but is not limited to: * Providing accurate information to referral sources for their use in assessing the needs of the patient and their ability to provide them. * Interfacing with referral sources and their on-site representatives * Facilitating completion of paperwork and obtaining required authorization signatures * Facilitating completion, distribution and signing of IM and patient choice letters * Arranging for patient transport (family pick-up, air ambulance, ground ambulance, taxi, bus) * Arranging for durable medical equipment and oxygen * Arranging for transfers to skilled and long-term nursing facilities * Arranging for transfers to assisted living facilities * Arranging for transfers to board and care facilities * Arranging for transfers to acute rehabilitation facilities * Arranging for transfers to respite centers * Arranging for transfers to shelters * Arranging for home health services * Arranging for home infusion services * Arranging for transfers to other acute care hospitals * Arranging for outpatient services * Arranging for wound vacs * Arranging for discharge medications to be paid for by the Medical Center * Obtaining patient clothing
Facilitates obtaining estimates/proposals/contracts with referral sources when the Medical Center will pay for post-acute care services for self-pay patients.
Assists Director with booking process.
Assists Case Managers with QIO appeals process. Maintains QIO appeals log up to date
Documents all interventions and necessary patient related activities in the electronic medical record
Other duties as needed.
