Summary

Provides essential support to the delivery of home health services in compliance with regulations and agency policies to ensure excellent patient care. Manages Admission/Discharge log and uses to ensure accurate patient census and receipt of all required documentation from clinical staff. Reconciles visits ordered to visits planned and made, and identifies variances for immediate resolution. Supports Clinical Manager in payroll, Hopkins One expense entries and report monitoring functions. Maintains and ensures the daily operation or organized, cost effective processes that are at or under budget. Manages and maintains an accurate patient schedule for each clinician. Ensures that staff productivity standards and agency admission targets are met or exceeded. Performs activities by effectively coordinating patient scheduling and admission information between HCC, Intake, Managers and Clinicians. Ensures that all patient visits are scheduled in conjunction with physician orders and payer authorizations. Responsible for re-review of clinical records and duties related to the same. Ensures that clinicians make corrections as designated by a CRN and follow up with the appropriate manager for issues of clinician non-compliance related to documentation. High school graduate or equivalent required.

Post high school education or training preferred.

Work Experience:
