Job Description
Mainstay United offers a supervised community-based residential program designed to provide a range of established and innovative rehabilitation strategies for adults with developmental disabilities and problematic sexual behaviors who are in need of clinical/forensic treatment. The program focuses on community and personal safety, accountability, competence, least restrictive alternative, normalization, due process and control.

The Program Specialist position holds the responsibility of supporting health, safety, and growth of the individuals in the program; including medical requirements, programmatic concerns, incident reporting and individual finances. A commitment to the RHD values should be demonstrated as job duties are performed.

RESPONSIBILITIES/DUTIES

Essential Duties and Functions

* Create an environment that empowers individuals through the development of skills and coordination of resources and supportive services

* Oversee the quality of programmatic, health and facilitative services by ensuring that the needs of individuals served are met and necessary services and supports are provided

* Advocate for consumer needs

* Meet with consumer s team to identify service needs and goals to ensure that such goals are accurately documented in the ISP

* Schedule, chair and document ISP meetings and consumer reviews along with SC. Monitor annual ISP progress for all consumers, along with SC s and other members of the consumer s team. Ensure all ISP s are accurate and person-centered

* Create, review and implement individual goals and data sheets based on assessment and ISP to monitor consumer progress

* Write instructional plans for the implementation of goals, objectives, and outcomes to implement consumer long and short-term goals and train all staff in the implementation of ISPs annually

* Participate in orientation of all TRS I and TRS II staff. Provide staff training as it relates to service delivery requirements and expectations as outlines in each consumers ISP.

* Update staff in a timely manner every time service expectations change to ensure that all staff are properly trained and up to date on consumer plans.

* Attend and actively participate in all team meetings

* Assist in the planning, writing and presenting of consumer case conferences and restrictive procedure plans

* Assist consumers in determining employment readiness and connecting with a job coach

* Maintain a minimum of 40-hours of in-service trainings per calendar year

* Organize, maintain and update consumer files in accordance with the DPW 6400 regulations

* Audit files at the office and sites every other month

* Monitor program records at all sites regularly

* Participate and coordinate licensing preparations and provider monitoring paperwork

* Complete and review annual assessments and progress summaries for all consumers, all monthly and quarterly progress notes and lifetime medical histories

* Ensure that all consumers medical, dental, psychiatric, and psychological services are scheduled in accordance with 6400 regulations and doctor recommendations

* Attend consumer medical and psychiatric appointments in the absence of Site Supervisor

* Ensure that all service delivery forms, individual goal sheets and data sheets and other forms as requires by the regulations, RHD, Division or program are completed in a timely manner and accurately reflect the consumer s individual needs.

* Maintain open communication with individuals, family members, day programs, Behavior Consultants, ISC s and all other outside entities

* Coordinate all monitoring visits of ISC s and other outside monitors

* Accompany licensors to the sites during annual inspections along with Unit Director

* Coordinate and distribute all consumer financial matters such as SSI, food stamps, rep-payee-ship, burial accounts, taxes, SSA Monthly Wage Reports, Room and Board contracts, renewal of annual stipends etc. with Fadmin

* Coordinate all communication for the consumers with the Social Security Administration, Medical Insurance Companies, and other outside entities

* Develop and implement a continuous quality improvement plan implementation and data collection in conjunction with Mainstay management team and Division leaders

* Develop and maintain a catalog of community resources/activities

* Attend house meetings at the sites on a quarterly basis

* Conduct regular unannounced visits of all Mainstay sites and share any necessary documentation

* Communicate any programmatic issues to Unit Director on a weekly basis

* Maintain a rotating on-call administrative status as part of the management team

* Serve as incident management point person and certified investigator

* Assist with opening new homes and transitioning consumers into, within and out of the program

* Complete all documentation necessary as related to programmatic changes properly

* Utilize Microsoft Word, Excel, and Power Point to complete reporting requirements and submit necessary data within a timely fashion

* Represent the Mainstay program in a professional manner and model leadership and boundaries with staff and consumers at all times

* Complete all other miscellaneous duties as requested or assigned

REQUIRED EXPERIENCE

Bachelor s Degree from an accredited college or university with a minimum of two years experience working with individuals with intellectual disabilities or Associates Degree with a minimum of 60 credits from an accredited college or university and five years experience working with individuals with intellectual disabilities.
