Job Description:

Overview

Purpose of the Job: Responsible for providing and assisting participants with personal care, activities of daily living, and basic therapeutic care necessary to the preservation of the home environment. Assists in monitoring and reporting the health status and providing a safe environment for the participants of Immanuel Pathways. Supports and lives out Immanuel's Mission and CHRIST Promises.

Requirements of Job:


  • Be legally authorized (currently licensed or if applicable, certified or registered) to practice the job's functions and actions in the respective state in which he/she is employed.

  • Only act within the scope of his/her authority to practice in the respective state in which he/she is employed.

  • Agree to abide by the philosophy, practices, and protocols of the PACE organization

  • Completion of the Home Health Aide Skill Competency Checklist and the Competency Skill written exam (passing grade of 80% or higher) prior to caring for participants.

Participant Care & Assistance 70%


  • Consults each day with the In-Home Services Manager (or designee) to be updated on Immanuel Pathways participant's status, changes in plan of care and to confirm scheduled visits.

  • Under the direction and supervision of the Registered Nurse assists participants in the home with the following duties:

    • Personal hygiene such as skin, mouth, hair care and bathing.

    • Assists participants in and out of bed assisting with ambulation.

    • Assists with prescribed exercise which participants and aide have been taught by appropriate personnel.

    • Prepares meals, including therapeutic diets, assists participants with meals.

    • Assists participants to the bathroom or in using commodes, bedpans, or urinals.

    • Assists participants with medications which are ordinarily self-administered.

    • Other duties taught by a health profession for a specific participant such as: taking of vitals, changing colostomy bags, changing of non-sterile dressing and non-sterile bowel and bladder hygiene care.



  • Performs directed household services.

  • Reports changes in participant's condition and needs to the In-Home Services Manager (or designee).

  • Completes appropriate documentation of services provided following Immanuel Pathways policy.


Compliance 10%

  • Demonstrate dependability through consistent compliance with scheduled work hours.

  • Comply with all policies and procedures.

  • Act within the scope of Job's authority.

  • Maintain the confidentiality of all company procedures, results and information about participants, clients or families.

  • Not perform any acts which require the exercise of nursing or medical judgement.


Safety Standards 5%
Follows safety policies and guidelines through:

  • Complying with OSHA (Occupational Safety and Health Administration) guidelines

  • Implementing Exposure Control Plan

  • Complying with Emergency Preparedness Plan

  • Using proper lifting and push/pull techniques, use gate belt assistance and participant transfers


Other 5%
Perform other duties as required or requested

Qualifications

Education-


  • High school diploma or equivalent.

  • Current License as a Certified Nursing Assistant in the respective state in which he/she is employed.

  • At Nebraska PACE locations: Be at least 18 years of age according to Nebraska State Regulation.

  • Complete a 75-hour course of training for certified nursing assistants.


Experience-

  • One (1) year of experience working with the frail or elderly population required, or completion of job specific training related to working with the elderly population must be completed within the first six months of hire.

  • One (1) year of experience is required working as a C.N.A in a nursing home, as a home health care worker, or similar position with seniors.


Other Requirements-

  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

  • Must have a valid driver's license, proof of insurance and have means of transportation.

  • Basic Life Support (BLS) Skills i.e. Health Care Level Cardiopulmonary resuscitation (CPR); Automated External Defibrillator (AED); First Aide (FA).


KSA- Knowledge Skills and Abilities-

  • Knowledge of principles, practices standards and techniques of a C.N.A.

  • Knowledge of common safety hazards and precautions to establish a safe work environment.

  • Knowledge of how to use and maintain medical equipment.

  • Skilled in establishing and maintaining effective working relationships with patients, co-workers, medical staff, and the public.

  • Skilled in identifying and recommending problem resolution.

  • Ability to work independently with minimum supervision.

  • Ability to communicate clearly and effectively.

  • Ability to react calmly and effectively in emergency situations.

  • Ability to chart via Electronic Health Records

  • Proven experience and basic computer proficiency (internet, email, Microsoft Office)
