Job Description:
The successful applicant for this position will enjoy a fast paced work environment helping individuals develop their strengths. We are a strengths based program working to identify individuals' areas of strengths and access their individual potential for growth within their natural settings. The work done with each client is individualized to the personal needs and strengths of each client. Wellness Coach will be responsible for care coordination, linkage, education, advocacy and referral for all clients. Wellness Coach will work with children and adult clients. They will work closely with the clinic staff, schools, courts and community supports in providing services for the client. This is a community based position which requires home visits (mileage reimbursed) and documentation of all work with the client is done via an electronic medical record. Laptop and cell phone provided.

Essential Duties and Responsibilities:
- Provides comprehensive case management to clients that are seeking mental health services.
- Interviews clients and/or their family members or other treatment providers, as needed.
- Provides appropriate referrals and follow-up as necessary.
- Prepares and maintains confidential case records and documentation in the electronic health record.
- Consults with service providers and community partners on resource-related issues.
- Work with and provide encouragement to clients to address and overcome barriers so they may be able to meet their physical and emotional/behavioral health goals.
- Ensures confidentiality and controls access to sensitive information.
- Collaborates with clinical team to perform care coordination.
- Develops network of trusted contacts within organizations in which to refer and coordinate care.
- Participates in multidisciplinary team meetings.
- Provides services via telehealth platform, as required.
- Achieves targets related to participant contact.
- Provides health promotion and assists participants and their families/caretakers in developing self-management skills to improve their health.
Job Qualifications:
- Bachelor's Degree in Social Work, Sociology, Psychology or related field required
- 1+ year's case management experience; 3+ years preferred. 2+ years in a customer service focused environment.
- Knowledge of and experience working with local social service, public health, and community resources.
- Must demonstrate leadership capacity and ability to foster team-building and a supportive, flexible environment.
- Excellent written and oral communication skills.
- Must be highly organized, detail oriented, and reliable.
Knowledge, Skills, and Abilities
- Ability to communicate clearly and effectively via oral or written means. Ability to present a friendly and positive demeanor to clients, their families, and staff. Ability to make oral presentations.
- Demonstrated ability to remain calm in the midst of difficult circumstances and emergencies. Flexible and able to adapt to change. Ability to respond in a professional manner in all situations.
- Working knowledge of Outlook, Word and Excel; PowerPoint a plus. History of documenting within an electronic health record preferred.
- Ability to think analytically and evaluate the impact of case management recommendations.
- Ability to set work priorities and to evaluate and create solutions to work related problems.
- Knowledge of case management services, community resources, and mental health.
