POSITION SUMMARY:
To provide social casework as an entity where appropriate to individual and families receiving care at Palms West Hospital. To coordinate the delivery of services to meet patients's discharge needs. To be capable and comfortable offering alternatives in patient care (Home Health, SNF, ALF, ect.). To facilitate timely and appropriate discharge from the hospital. To provide documentation of all functions as required by regulatory agencies.
QUALIFICATIONS
* Masters in Social Work
* Minimum of two years experience in Medical Social Work, preferably in a medial setting with the ability to work with Neonates, Pediatrics, Adolescents and Adults
* Previous experience in an acute hospital setting required
* Knowledge of Federal and State guidelines and requirements: Health Care District, Medicare, Medicaid and Department of Children and Families
