Seeking a SOCIAL WORKER for our practice who provides clinical expertise to ensure all patients receive high quality, efficient care. The Social Worker:
* Provides direct social service and counseling to patients/families
* Assists in locating appropriate resources for post-discharge care
* Participate in multidisciplinary team to coordinate the client's plan
* Cross-trains with team members to serve as back up resource
* Assesses and identifies cases for intervention
* Prioritizes goals and develops plans based on client needs
* Monitors and evaluates outcome of client's plan
* Proactively identifies cases
* Collaborates with third party payors internal case management/disease management departments as appropriate for discharge planning needs
QUALIFICATIONS
EDUCATION:
* Bachelors degree or Masters degree required
LICENSURE/CERTIFICATION/REGISTRATION:
* Current licensure by Texas State Board of Social Work
* CMSW, LCSW or LCP preferred
* BLS issued by the American Heart Association
EXPERIENCE:
* One year social work experience preferred
