A unique opportunity for a Nurse Director of Outpatient Case Management has just become available at a reputable hospital in scenic, Northern California.

This 300+ bed hospital is seeking a RN Director of Care Coordination Skilled Nursing Facility to join their team. This hospital has been serving their community for over 100 years and is part of a larger healthcare system. It has been voted one of the best places to work for many years.

The experienced Nurse Director of Case Management will be a leader to case managers, social workers, and case management coordinators. They will collaborate with hospital administration and will generate systems of coordination between Access Care Services, Business Office, Medical Records, Nursing Units, Emergency Department, Bed Control Social Services to increase the success of the case management processes. The RN Director of Care Management will create communication between services using continuous quality improvement tools.

The Nurse Director of Case Management Ambulatory will oversee the activities of the Case Management, Social Services and Denial Management/Recovery Audit Contractor/Nurse Auditing for outpatient programs. Their responsibilities include annual budget development, new program development, establishing department policies and procedures, and maintaining quality assurance. The RN Director of Care Management Skilled Nursing Facility will work with the Administrative Team and Utilization Review Committee to set financial goals.
