Position Description


Einstein Medical Center Montgomery is a medical destination providing many new healthcare services and resources previously unavailable in the area including advanced medical and surgical care, diagnostic testing services, a neonatal intensive care unit, and 24 hour emergency care. In this role you will be responsible for:
- The primary function of this position is to support and expand the patient-physician partnership.
- The RN will assist in leading the quality team in working with and supporting the providers and practice staff to demonstrate improvement in Population Health and QM initiatives designated by ECP Quality Strategic Plan to improve the health and wellness of our patients
- This position will assist in the delivery of continuous and comprehensive primary care within the medical home.
- The RN will work collaboratively with a care team of providers, pharmacists, social workers, behavioral counselors, and nurses.
- It will address the health maintenance needs of all patients and the chronic care needs of those with one or more the clinically important conditions identified in the practice.
- The RN Clinical Care Coordinator will focus on population management, patient self-management support, transitions in care, and resources connection
- Specific Duties required for RN includes: Chronic Care Management Services (CCM) and Care Plan Through the use of the RN Standards of Practice, the RN will develop and maintain a patient-centered Care Plan based on a physical, mental, cognitive, psychosocial, functional, and environmental assessment, and an inventory of resources as needed
- The RN will work/support the providers and office staff to reach expected quality goals.
- The RN Clinical Care Coordinator will provide direction/supervision to the LPN Clinical Care Coordinators regarding quality initiatives and departmental/practice goals
- The RN Clinical Care Coordinator will assess patients healthcare needs based on clinical presentation, medical diagnosis and patient presentation.

If you possess the following qualifications, please apply immediately:
- Bachelor's degree in Nursing preferred
- Current Pennsylvania RN license, active and in good standing is required
- Minimum 2 years of clinical experience that must include ambulatory physician office experience or acute care setting preferably medical/surgical.
- CPR certification is preferred
- Excellent interpersonal communication and negotiation skills
- Clinical competence in disease management and case management principles
- Clinical competence in applying techniques of assessing psychosocial, behavioral and psychological aspects of a patients problem
- Competence in establishing rapport with a patient and community agencies
- Strong organizational, analytical, time management, and computer skills
- Ability to work independently
- Demonstrates the knowledge and skills necessary to provide care for the infant, child, adolescent, adult and senior citizen
- Ability to elicit information to make assessments, identify expected outcomes and evaluates progress toward attainment of goals and outcomes. Physical Demands:
- Physical Requirements: Sitting, walking, reaching, pulling, bending, stooping, twisting, standing, lifting (up to 10 lbs.). Manual dexterity for operating computer and clinical equipment. Periodic visits to community agencies is required. Requires frequent verbal and written communication in English.
- Visual Requirements: 20/20 vision (with/without corrective lenses).
- Hearing Requirements: Ability to hear normal conversations.
- Working Conditions: Normal ambulatory office clinical settings with high activity
