Commonwealth Care Alliance's (CCA) Care Planning Unit (CPU) is primarily responsible for developing and ensuring adherence to high quality, member-centric care plans through timely execution of community-based assessments for CCA members and ongoing tracking and monitoring.

The Supervisor, Regional RN Care Planning supervises a team of site-based nurses responsible for conducting initial and ongoing assessments and creating patient-centered care plans. The Supervisor, Regional RN ensures that Minimum Data Data (MDS) assessments are completed and documented in a timely, comprehensive, and contractually-compliant manner and that care plans are developed in a robust, member-centered manner.

The Supervisor, Regional RN Care Planning reports to Clinical Director, Care Planning.

What You'll Be Doing

Hires assessment nurses and provides ongoing training regarding standard operating procedures and quality assessments and care plans; tracks and monitors team productivity, and as needed, secures per diem staff to supplement team.
- Maintains expert knowledge of the MDS-HC form and how it is used to measure function leading to rating categories that support member needs and related costs
- Ensures adequate training and technology is made available for nurses conducting MDS visits.
- Works closely with Care Planning Unit managers to balance nurse workload and ensures that assessments and care plans are being done on time and are of consistently high quality
- Conducts a limited number of in-person MDS visits, developing and disseminating best practices regarding MDS completion and patient assessment experience.
- Participates in member assignment stratification meetings with the team and follows up to ensure that members are assigned to the apprropriate clinical model
- Continually improves expertise to optimize capture of correct rating category
- Regularly collaborates with other Care Planning Unit Managers to improve workflow and attain team objectives
- Reviews MDS's for training and quality assurance purposes and collaborates closely with the Manager, MDS Review to improve skills of clinicians
- Arranges for transfer of cases between nurses when needed due to scheduling issues.

What We're Looking For

- Associate's Degree in Nursing
- Bachelor's Degree Preferred
- 5+ years clinical experience
- Experience completing MDS assessments and care plans
- Prior supervisory experience preferred
- RN with active registered nursing license in Commonwealth of Massachusetts, in good standing
- Ability to triage urgent clinical issues and make independent decisions
- Excellent organization skills
- Ability to function effectively as part of a team
- Effective oral and written skills
- Strong interpersonal and customer relations skills
- Effective teaching skills
- Good to excellent technology skills, including use of electronic health records and Microsoft Excel
- Ability to model and create a positive work environment
- Experience with electronic medical record strongly preferred (eCW a plus)
- English required, bilingual preferred
