Job Description:


Primary Purpose:

The Case Manager utilizes advanced nursing skills and knowledge of resource management and fiscal responsibility to coordinate the clinical care for a designated patient population across the continuum of care. The responsibilities include but are not limited to clinical effectiveness, discharge planning, care coordination, and clinical resource management. The Case Manager interacts with Medical, Nursing and Ancillary department staff to facilitate quality based, cost effective patient outcomes and ensure care is provided in the most appropriate setting.

EDUCATION:

  • Registered Nurse from an accredited school of Nursing, College or University required. Bachelor of Science in Nursing (BSN) preferred.

LICENSE & CERTIFICATION:


  • Current RN License in Ohio


  • Certification in Case Management or Certified Professional in Health care Quality (CPHQ) encouraged.


  • BLS (American Hearth Association),and current at time of hire.


YEARS OF EXPERIENCE:

  • Three years Home Health, Discharge Planning, and / or Case Management experience.

REQUIRED SKILLS AND KNOWLEDGE:


  • Knowledge of level of care management, discharge planning, home health care, and case management required; basic knowledge of third party payers preferred.


  • Knowledge of and ability to apply clinical decisions and medical necessity criteria.


  • Ability to effectively communicate with and gain cooperation of resident and attending physicians with regards to documentation needs and to effectively influence all members of the interdisciplinary care team during difficult cases to achieve appropriate resolutions.


  • Effective written and verbal communication skills required for establishing and maintaining relationships.


  • Ability to interpret medical data as documented.


  • Strong organizational skills to manage multiple, complex activities with tight deadlines, establish priorities and manage time effectively.


  • Effective conflict resolution and negotiation skills.


  • Knowledge and training in multiple computer systems required to perform position responsibilities (i.e. LastWord, Microsoft Office, etc.).


  • Demonstrates initiative and responsibility for professional nursing practice.


  • Maintains excellent customer service attitude.
