Job Summary:

* You will provide assessment, counseling, community service coordination and assists with discharge planning to facilitate continuity of care.

* You will investigate, assess and plan activities to help patients cope with social, emotional, economic and environmental problems.

* You will provide direct social work services to help patients regain independence and to live as normal a life as possible in the community and to advise health service personnel on social matters affecting present and future health care.

* You will participate with Transitional Care team to provide social work services for skilled nursing patient population. Care for Neonate/Infant (0-12 months), Pediatrics (1 - 17 years), Adults (18 - 79 years), Elderly (80+ years).

* You will be responsible for maintaining standards of professional and ethical practice in the provision of the full scope of social work services.

QUALIFICATIONS

* Bachelor or Masters degree in Social Work

* 2 years experience in mental health field and/or acute care environment

* Current BLS or obtained within 90 days of hire

* Knowledge of available community resources

* Ability to take evening and weekend call

* Prefer 3 years acute care facility experience
