RN - Level of Care Enrollment at St. Paul's Senior Services in Encinitas, California

Job Description:

St. Paul’s Senior Services is excited to announce an amazing opportunity to be part of San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. 

This position will work at two sites - Encinitas and Downtown San Diego.

 

Job Summary:


Under the direction of the Enrollment Supervisor, provides medical and lifestyle assessment to clients who are enrolling in St. Paul’s PACE both in-home and in the PACE center. Documents assessment in DHCS tool tracks the progress of DHCS approval process, responds to DHCS questions, reports approvals to supervisor, documents denials and organizes appropriate documentation for denials per policy. Meets with IDT as needed to discuss extended LOC recommendations and home observations. Documents and counts medications for reporting to clinic staff.

Qualifications and Requirements:

• 
  Education - Graduate of a school of professional nursing. Current California Registered Nurses License, CPR certification required, BSN preferred. Shall be at least 18 years of age.
  


• 
  Experience – A minimum of one year’s experience in working with the frail or elderly population required. Shall have either training or related experience in the job assigned. 
  

Physical Requirements:


Shall be in good mental and physical health, and capable of performing assigned tasks. Requires a full range of motion including handling and lifting patients, manual and finger dexterity and eye-hand coordination. Requires standing, walking, pushing, bending, kneeling and reaching at arms length and overhead in clinic, rehab department and in community for prolonged periods of time. Must be able to transfer participants needing maximum assist of one. Requires the ability to lift/carry up to 50 pounds and push or pull objects exceeding 150 pound, using appropriate body mechanics. 

YOU MUST BE ABLE TO CLEAR FINGERPRINTS THROUGH DOJ AND FBI IN ADDITION TO A DRUG SCREEN CLEARANCE.
