Job Summary
The Home Care Clinical Nurse is responsible for providing nursing care and services to home care patients in accordance with the CHOP, CHHC standards of practice, policy and procedure and within all regulatory and accrediting agency guidelines. Utilizes the nursing process to deliver nursing care to patients and families.
Job Responsibilities
- Practice: Provides competent care for patients and families based on experience.
- Nursing practice and documentation is in accordance with CHOP, CHHC standards of care, policy &procedure as well as all regulatory and accrediting agency guidelines.
- Participates in on -call rotation to meet patient care needs.
- Professionalism/Leadership: Demonstrates competence in professional practice.
- Research: Demonstrates an awareness of research and its value to clinical practice.
- Participates in efforts to apply research findings to practice.
- Learning Orientation: Participates in the development of professional and educational activities.
- Examines successes and failures to improve performance.
- Maintains a blame free environment.
- Able to identify own learning needs. Seeks out opportunity to meet needs and attends all required programs.
- Resource Management: Fully understand appropriate utilization of financial resources.
- Participates in promoting quality, efficient, and cost effective care.

Required Licenses, Certifications, Registrations
RN - PA
RN License to practice professional nursing in New Jersey within 9 months of hire required for Full/Part time status if position is based in NJ.
Required Education and Experience
- Minimum education: Nursing Diploma
- Pediatric acute, subacute, other inpatient or Home Care experience
Preferred Education, Experience & Cert/Lic
Preferred Education: Bachelor's degree in nursing
Preferred Experience: Two (2) years of inpatient nursing with pediatrics/adults or at least one(1) year of home care experience
Additional Technical Requirements
- Valid Driver's license
- Strong interpersonal and customer service skills
- Strong verbal and communication skills
- Strong clinical judgment skills
- Ability to work independently
- Ability to work as a team player in a fast paced environment
- Data entry skills
