Job Description:

The YNHH Comprehensive Stroke Center serves as one of two Joint Commission-certified stroke programs in Connecticut. As such, we serve as the region's destination hospital for complex stroke patients that would benefit from advanced treatment options and complete stroke recovery care including secondary prevention. A major need and current gap is establishing ongoing relationships with the stroke programs within the delivery network as well as with programs outside the network that routinely transfer patients to YNHH. These relationships range from building and maintaining partnerships with telestroke \\"spoke\\" hospitals to collaborating ith other community hospitals that elect to refer complex stroke patient for neuro-interventional or neuro-surgical interventions. Additionally there is a requirement and benefit to maintaining liaison-ships with the multitude of EMS services that serve the greater New Haven County and selected secondary catchment areas. As such, we are proposing an FTE entitled, \\"Outreach and Integration Coordinator\\" (OIC) who would be responsible for \\"all activities\\" pre-hospital. The Stroke Program OIC would be responsible for supporting the liaison work, education, and performance feedback to the multitude of EMS teams within the Centers for Emergency Medical Services as well as those systems outside this Service (including both air and grond transport operations) that transfer stroke patients to YNHH. The OIC would service as the clinical liaison to the telestroke program under the leadership of the telestroke medical director. Additionally, the OIC would oversee the regulatory and performance requirement related to the above entities as required by the Joint Commission.

RESPONSIBILITIES

* 1. Clinical Coordination of the pre-hospital activities for hyper-acute stroke care 1.1 Liaison with Region 2 EMS and facilities that transport patients to YNHH fro outside Region 2 including Life Star and Sky Health 1.2 Responsible for generating and distributing feedback forms to all facilities that transport acute stroke patients to YNHH for a higher level of care 1.3 Attendance at 50% of the CEMS meetings each year; and additional EMS meetings outside CEMS as warranted 1.4 Integration of the Stroke Program-approved transport protocols with all stroke-specific transfer protocol 1.5 Develops and implements a process for collecting relevant clinical data that serves as a proxy for clinical effectiveness and quality of care

* 2. Pre-Hospital Regulatory Readiness 2.1 Serves as a liaison with the Stroke Program Medical Director and Stroke Program Manager of the Comprehensive Stroke Center (and program affiliates) to ensure that pre-hospital practice standards are in compliance with the Joint Commission requirements 2.2. Works with the EMS directors to achieve a consistent process for ensuring that EMS run-sheets are integrated into the electronic medical record 2.3 Reviews EMS protocols for the pre-hospital care of the suspected stroke patient and ensures that they meet criteria set forth by the latest AHA/ASA guidelines 2.4 Oversees the Door-in; Door-Out data collection process for intra-facility transfers 2.5 Responsible for the Shoreline and St Raphael's campuses intra-facility practices for transporting a suspected stroke patient to the York Street campus 2.6 Oversee the initial and renewal neuroscience specific transfer agreement for any hospital that transfers neuroscience patients to YNHH

* 3. Clinical liaison for the Comprehensive Stroke Center's Telestroke Program: Reports to the Stroke Program's Telestroke Medical Director 3.1 provides clinical nursing expertise to hospitals signed up to participate in the hospital's Telestroke services (initial orientation, education, regulatory/certification requirements, and feedback). 3.2 Tracks selected volumes, practices, documentation, and both patient and system-related data for both process and outcomes review 3.3 Participates in both HUB and SPOKE organizational meetings; interfaces with theIT department to facilitate a smooth transition to telestroke hospital to incorporate technology into clinical practice

* 4. Education: 4.1 Reviews/contributes to the pre-hospital and telestroke education modules to support pre-hospital and nursing practices go hyperacute stroke care. 4.2 Serves as a liaison with the neurosciences clinical educator and the stroke program's team leadership to ensure that the education and training for care of the pre-hospital and hyperacute patient is in compliance with the most current stroke practice guidelines 4.3 Provides opportunities during the year to review and approve the educational requirements for ongoing learning and competence of the medical and nursing staff at YNHH 4.4 Assists with the coordination of educational opportunities across the delivery networks

QUALIFICATIONS

EDUCATION

Masters' degree in Nursing required; leadership certifications preferred

EXPERIENCE

Minimum 3-5 years of neuroscience nursing, preferably in vascular neurology and/or neuro-critical care; SCRN, CCRN, CNRN certifications a plus

LICENSURE

RN, MSN, ACLS required; SCRN required within the first two years of hire

SPECIAL SKILLS

Excellent interpersonal communication skills, effective time management planning within the larger program, effective problem solving and capacity to participate in both oral and written communication venues. Robust knowledge and practice related to the neuroscience service line (including both acute and recovery stroke care) and the associated standards/requirements set forth by CMS, DPH and the Joint Commission. Able to negotiate and flex to the requests and needs of the various outside hospital stroke programs; local and regional travel is required in order to provide services and support to practice locations within and outside the delivery networks. Microsoft Office/Excel and computer skills required
