Job Summary:

* Assist home friend manager with the Home Friend Program performing in-home assessments, provide case management services to clients and Assist with home friend staff.

Qualifications:

* High School diploma with three years of progressive responsibility in case management of older adults and disabled.

* Must be computer literate and posses very strong inter-personnel communication skills.
