Job Summary:
The Care Coordinator assists patient/families with psycho social, cultural and financial considerations that may impede recovery. Formulates plan for post-hospital care. Demonstrates knowledge of human growth and development, assessments, range of treatment, and care of patients appropriate to the ages of patients served.
The position's tasks and responsibilities include:
* You will be a supportive lead role to peers and Director of Case Management.
* You will audit information collection for DMat meeting.
* You will participate as a member in the Utilization Review Committee.
* You will be a clinical resource for staff.
* You will assist with prior authorizations for medications.
* You will assist with Observation review list and progressions as indicated.

QUALIFICATIONS
* Graduate of an accredited nursing school program.
* RN license required in an approved compact state.
* 1-3 years' experience in an acute medical setting.
* Strong clinical assessment skills.
* BLS certification. New employees have up to 90 days from date of hire to obtain the American Heart Association BLS for healthcare providers.
