The Ambulatory Pharmacy Liaison provides improved service for Hackensack Meridian patients by helping to supply medications prescribed by the patient's physician for discharge in a timely manner so that the patient can be discharged efficiently from an acute care Meridian facility and transition directly to home. The Liaison functions as an intermediary between the discharge patient and the ambulatory pharmacy.
The Liaison will work closely with floor nurses, discharge planners, physicians, and the ambulatory pharmacy team. The Liaison helps ensure patient educational needs are met relative to their discharge prescriptions by facilitating pharmacist counseling where necessary.
Responsibilities
The Liaison meets with all potential patients to describe the program and answer any questions the patient or their caregiver may have. For patients requesting to use the service, the Liaison assists with obtaining information necessary for prescription processing, helps prioritize workflow and facilitates delivery of completed prescriptions back to the patient prior to discharge.
The Liaison, as a registered pharmacy technician may also work as a technician.
Qualifications
High School Diploma or equivalent required
- NJ Registered Pharmacy technician REQUIRED
- A minimum of one year experience in a retail pharmacy or ambulatory pharmacy setting is required, two or more is preffered
- PTCB Certification (Pharmacy Technician Certification Board) preferred
- Strong knowledge of third party programs and demonstrated ability to exceed customer expectations
- Strong interpersonal communications skills; compassionate and empathetic; passion to succeed while helping others; takes initiative (self-starter)
- Pays attention to detail and can document outcomes; strong ability to multi-task
- Computer and technology literacy above average
