Position Summary:

LVN to do Continuous Care visits on the dying patient which involves 8 hour shifts providing symptom management under the direction of the RN. LVN visiting nurse to perform visits of Hospice and or Palliative patients in their home to perform assessment of patient's condition, patient teachings and perform nursing procedures. Follows plan of care under the direction of the RN. Experience:

• One-year Practical experience as an LVN and knowledge of home health and or hospice, excellent computer skills.


• Time management and organizational skills, attention to detail, and a focus on completion of tasks, are critical.

• Minimum of 2 years as an Licensed Practical Nurse (LVN)


• BLS CPR required


• Current California license


• Have dependable transportation, driver's license and car insurance
