Our program strives to treat the whole patient by seamlessly integrating care for patient’s medical and behavioral care needs within our practice. The Primary Care Addiction Nurse Practitioner will provide nursing and medical services to individuals, families, and groups in the setting of a patient-centered medical home for a diverse population as part of a recovery program with a multidisciplinary team, including primary care, behavioral health, psychiatric services, and other support services in order to sustain long term recovery. The candidate will diagnose and manage substance use disorders along with other acute episodic and chronic illnesses while emphasizing health promotion and disease prevention. Services may include ordering, conducting, supervising, and interpreting diagnostic and laboratory tests, and prescribing pharmacologic agents, non-pharmacologic therapies, and support services.Essential Duties & Responsibilities:
