Medical Command Center (MCC)

The MCC is a clinical call center that utilizes a national network of care coordinators and communication centers to link 911 call centers, nurse navigators, physicians, and mobile medical response teams together to provide world class care coordination, patient navigation and unplanned care services.

 

Our fully licensed and specially credentialed staff of nurse navigators answer calls that come into our MCCs in Dallas, Texas and Pompano Beach, Florida 24/7, 365 days a year. We use a proprietary evidence based clinical decision software to immediately evaluate a patient’s current condition and provide a needs matched time appropriate resource allocation (NMTARA) to meet the patient’s unique healthcare needs. Our five-level proprietary triage system ensures we are delivering the right resource at the right time, in the right setting to achieve the right outcome at the right cost.

Summary

Access2Care programs are designed to utilize the appropriate provider, tools and technologies required to best meet the various healthcare needs of patient populations. Access2Care is seeking to hire dynamic, experienced licensed vocational nurse to remotely triage, monitor and provide care navigation to a variety of members and patients. As an integral part of the MCC your efforts will be supported by innovative communications and logistical management platforms that are specifically designed to improve large patient population’s access to care, experience, and clinical outcomes. This position presents the opportunity to be a part of a new, exciting and innovative healthcare delivery model in the pre-hospital setting.

 

This role has primary responsibility to link mobile resources together, integrate and coordinate them to respond appropriately to patient needs through telephonic triage and remote patient monitoring. Serve as a coordinator in communication center to link 911 services and mobile clinical care teams utilizing innovative communications and logistical management platforms.

 

Essential Job Functions


  • Serves as Communications Center Supervisor and liaison with healthcare partners

  • Works with Access2Care’s Integrated Healthcare Program and partners to develop new avenues of patient pathways to access healthcare

  • Creates and maintains a positive and cooperative working environment in a virtual setting

  • Participates in activities that promote Access2Care Integrated Healthcare Program and Access2Care organization at large

  • Participates in quality assurance review of patient Inbound/Outbound calls and patient satisfaction surveys

  • Communicates with physicians and other healthcare agencies

  • Shift Supervisor in virtual setting providing oversight to Nurse Navigators and Non-Clinical Call Takers

  • Triages and assess patients, coordinating care needs across health care continuum

  • Coordination of Occupational Medicine services for employees injured at work

  • Surveillance and monitoring Medical Clearance forms for new employees

  • Oversight telemonitoring services in variety of settings

  • Provides instruction to patients regarding health and treatment plans

  • Lead and facilitate Integrated Healthcare training course provided by Access2Care

  • Prepares and reviews documentation, agency reports and statistical data

  • Participates in training, staff development, and quality assurance activities

  • Constantly demonstrates and maintains a high standard of confidentiality

  • Assist in community resource development

  • Assist in the development of Integrated Health Procedures/Flow maps

  • Ensures resource list is up to date and has a specific flow process with each

  • Assist in development of new protocols

  • Understands challenges and demands of Healthcare reform

  • Provides excellent Customer Service

  • Perform other duties as assigned by Access2Care Leadership

Knowledge:


  • Emergency clinical practices, procedures and protocol

  • Experienced with all patient ages, from pediatric to geriatric

  • National geography

  • General knowledge of computer

Technical Skills:


  • Type at least 30 words per minute

  • Use of Word and Excel

Behavioral and Physical Skills:


  • Act calmly and quickly in emergency situations

  • Create and maintain a positive and cooperative working environment in stressful situations

  • Be able to handle multiple tasks

Certification(s) and/or Licenses(s):


  • Active RN license without restriction

  • Must be willing and able to achieve licensure in other states and countries

Education and/or Experience:


  • Graduation from accredited nursing school

  • Experience with telephonic triage preferred

  • Experience with remote patient monitoring preferred
