As a case manager, you must focus on using strategies that are fiscally responsible, meaning they know how and where to get the necessary resources to deliver excellent care at a cost that is affordable. You will work together with patients and their families, but also with other physicians and partners such as the pharmaceutical and medical technology industry, as well as with insurance companies

About this unit:

  • You will assist in educating patients and their family members and making sure they have the resources and tools to be discharged.

 

Responsibilities:


  • The role of the RN Case Manager is to assess each assigned patient referral to identify discharge planning needs, and collaborates with patient, family, significant other, and members of the health care team to develop and implement comprehensive plan of care and discharge plans.

  • Balances individual clinical and psychosocial patient/family needs with the efficient and cost effective utilization of resources, while promoting quality outcomes through the continuum of care.

  • Reviews and performs utilization review functions on each assigned patient per hospital policy and procedure.

 


Qualifications



  • Graduate of accredited Professional School of Nursing, BSN preferred

  • Must hold a current RN license from the Kansas Board of Nursing or an active compact license. Must have excellent interpersonal and organizational skills. Ability to work effectively with health care team required. Knowledge of computers, statistics and managed care concepts preferred.

  • Minimum of three years medical/surgical/critical care experience required. Case Management/ UR experience preferred.
