PPOSBC has a brand new opportunity for a full time RN Case Manager/Surgical RN to join us!

This position will provide centralized case management including follow up care in accordance with Planned Parenthood regulations. Monitor follow up of abnormal labs, diagnostic studies, laboratory test results, and external care referrals in accordance with Planned Parenthood regulations and protocols and Board of Registered Nursing. Assist in management and follow-up of high-risk patient cases as requested by the Medical Director. Provide back-up and support for case management functions that must be performed by a licensed nurse, including follow up of all positive lab results assigned to the case management department. Participate in activities related to incident reporting including external reporting to ARMS. Provide support to the clinical staff by receiving calls from patients with clinical concerns, when clinician is not immediately available. Provide family planning, prenatal, and/or abortion services to patients in accordance with Board of Registered Nursing and Planned Parenthood regulations and protocols. Provide quality patient care by providing contraceptive, prenatal and abortion education and assessment.  Facilitate referrals to internal and external referral sources.

Responsibilities

Essential Functions:  Essential functions encompass the required tasks, duties and responsibilities performed as part of the job and the reason the job exists.


  • Provides standardization of centralized patient case management of abnormal and normal labs, telephone encounters and follow-up for all affiliate centers according to PPFA and PPOSBC’s Medical Standards and Guidelines. Ensure compliance with PPFA protocols for follow-up.

  • Report incidents through the AIMS reporting system as assigned. Update log of incident reports as entered.

  • Review Melody schedule to identify patients that did not show for their appointment and initiate follow up if needed.

  • Answer health related questions from patients during business hours.

  • Schedule appointments and screen as needed for family planning/primary care including but not limited to contraceptives and abortion services as needed.

  • Provide medication abortion follow up.

  • Assist in management of high-risk cases including expediting the referral process for high-risk case, assistiing the patient in identifying funding for their care, and reviewing the scanning records of evaluations sone outside the affiliate.

  • Cross train in additional case management tasks and provide coverage and assistance to other members of the case management team as needed.

  • Maintain electronic logs related to Incident Reporting, triage calls, and follow-up.

  • Attend meetings at both on and off-site locations as needed.

  • Collaborate with Clinicians/Providers as needed to ensure quality care regarding patient follow-up – including but not limited to scheduling appointments, coordinating outside referrals and outcomes.

  • Keep Clinician/Supervisor apprised of any problems, incidents or potential problems involving patients or staff as relates to any services.

  • Adhere to agency’s policies, procedures, and protocols in regards to performing physical exams, procedural skills, judgement, charting and communication with patients.  This may include successfully completing agency, regional and/or state certification requirements.

  • Adhere to all OSHA requirements.

  • Provide clinical and surgical services as outlined in the PPOSBC Medical Standards and Guidelines, Standardized Procedures, and Policies and Procedures, and any other guidelines recommended by the affiliate.

  • Participate in a team approach to patient care, and deliver care in a professional, confidential, and expeditious manner.

  • Act as resource person to medical staff.

  • Adhere to affiliate goals and policies on professionalism, wait time in-clinic and on the system for addressing client complaints.

  • Other duties as assigned by Supervisor/Director of Quality Management

Heath Center Duties:


  • Meet or exceed customer satisfaction survey benchmarks as set by the Patient Services Administration team.

  • Participate in health center efforts to achieve established goals for productivity, cycle times, and wait times.

  • Document patient data accurately and completely in the medical record.

  • Dispense/administer medications and contraceptive devices as per physician/clinician or standing orders.

  • Provide information and education regarding reproductive health care; family planning, pre-term labor, labor and delivery, breast feeding, postpartum care, abortion procedure (surgical and medical) and discharge instructions.

  • Monitor lab reconciliation process per policy. Review lab reports and initiate follow-up. Provide laboratory tests as needed, including phlebotomy.

  • Manage emergency situations as they arise in the center and consult with other licensed providers as necessary.

  • Daily performance of controls and completion of control logs for moderate complexity tests.

  • Daily monitoring and recording of medication, controls, and specimen refrigerators for temperature accuracy.

  • Manage medication inventory including ordering, confirmation of expiration dates when stocking and administering medications.

  • Assist with clinic upkeep: sterilization of instruments, clean lab and exam rooms, set up and restock rooms as appropriate.

  • Complete and submit incident report when necessary.

Surgical Duties:


  • Provide continuity of care for abortion services in a patient centered flow environment.

  • Counsel and perform physical exams on abortion patients.

  • Review medical history and perform pre-operative assessment

  • Perform Ultrasound

  • Perform appropriate lab work

  • Insertion and removal of IV line

  • Provide contraceptive counseling

  • Provide conscious sedation

  • Perform recovery room responsibilities

  • Review discharge orders and counsel patient on discharge instructions

  • Assist Sedation Provider with opening and closing narcotic inventory management

  • Assist Physician and other licensed staff as required



Qualifications

Licensure and/or Certification Requirements:


  • Current California Registered Nurse’s License required.

  • CPR and ACLS certification required.

Minimum Education:


  • AS/BS in Nursing required.

  • Preferred Maternal Child Health (MCH) education, including family planning.

Minimum Work Experience:


  • One or more years of ambulatory, reproductive health care, prenatal, and/or abortion services, including case management preferred.

  • Familiarity with CDC guidelines and state regulations related to STIs and other infectious diseases.

  • Working knowledge of the Case Management personnel daily functions.

  • Maintain professional demeanor at all times.

  • Excellent written and verbal communication skills.

  • Ability to demonstrate mature judgment, initiative and critical thinking.

  • Accuracy and attention to detail essential.

  • Availability to work flexible hours, as required.

Language Requirements/Preference:


  • Bilingual in Spanish is preferred.
