ESSENTIAL FUNCTIONS


 



  1. In collaboration with physicians and clinician colleagues, participates in the provision of direct and indirect patient care utilizing the nursing process including nursing assessment, problem identification and intervention.


  2. Receives patient problem telephone calls and performs nursing assessment, identifies patient problem, collaborates with physician to plan intervention, communicates instructions and education to patient and documents in electronic medical record. Ensures resolution of issue with consistent and clear follow up.


  3. Coordinates prior authorization requirements between patient, testing location, insurance company and physician.


  4. Coordinates prescription refill review and electronic ordering processing.  Schedules required patient appointments, as needed.


  5. Coordinates with support staff to schedule same day urgent patient appointments within the practice, emergency department, urgent care center or physician specialist office.



  6. In collaboration with physician, provides patient with diagnostic test results by telephone and/or electronic secure messaging portal and documents in electronic medical record.



  7. Administers physician-ordered medications and immunizations, as needed.


  8. Coordinates anticoagulation therapy management with physicians, communicates dosage and laboratory testing schedule to patient and documents in electronic medical record.


  9. Performs point-of-service INR testing and anticoagulation management as needed and documents in electronic medical record.


  10. Monitors and coordinates electronic secure patient portal messages and documents in electronic medical record.


  11. Monitors and coordinates electronic clinical messaging system and documents in electronic medical record.


  12. Coordinates patient home care services and required paperwork including VNA, IV Therapy, Elder Services and DME.


  13. Obtains and communicates critical laboratory and radiology results to physician, communicates to patient with instructions, as needed and documents in electronic medical record.


  14. Coordinates post-hospital, ER, SNF and other outside facility appointment scheduling and documents in electronic medical record.


  15. Assists with required form completion and disposition.


  16. Coordinates and communicates patient transfers to emergency department.


  17. Communicates and coordinates high-risk patient care with the iCMP team, as needed.


  18. Reviews and documents advanced directives in electronic medical record.


  19.  Assess unscheduled patients arriving with an illness or medical problem following practice triage protocols.



  20. Reviews and contacts patients who cancelled same day illness visit and document patient status in electronic medical record, as needed.



  21. Participates in all relevant hospital and practice programs and protocols as assigned.


  22. Assesses and documents patient TB test results in electronic medical record.


  23. Complies with all aspects of HIPAA and Information Security Plan regulations.


  24. Complies with all local, state and federal guidelines that govern nurse practice and licensure.


  25. Demonstrates excellence in quality patient care and quality service.


  26. Demonstrates effective coordination and communication skills between clinical and non-clinical staff.


  27. Acts as a resource person for staff and seeks guidance from supervisor, as needed.


  28. Plans personal professional development.


  29. Maintains neat and orderly workstation.


  30. Performs other duties, as assigned.

Graduate of an accredited school of nursing.


Minimum of one (1) year   LPN experience, in Cardiology preferred. Experience in outpatient ambulatory setting preferred.


 


 


 



Current licensure as a Licensed


Practical Nurse in the Commonwealth of Massachusetts required
