Case Manager - Community Based Services at Catholic Charities in Fort Meade, Maryland

We are currently looking for a Case Manager at Sarah's House. Sarah's House serves individuals faced with many challenges including mental health and substance abuse issues. This position is responsible for providing intensive casework services to clients of Sarah's House. The Case Manager will assess individuals’ needs, connect them with resources based on their goals, and provide support, advocacy, information, and referral services. Acts as a liaison with various agencies including, but not

limited to, Department of Social Services (DSS) and Veteran Administration (VA). Works collaboratively with Sarah’s House team and external providers to assure that families are receiving person centered services.


The Case Manager is responsible for:


  • Assures that each guest receives services in a dignified and respectful manner and in accordance with the

    program's philosophy.

  • Provides information and referral sources for guests and acts as an advocate/liaison with support services and

    resources.

  • Provides information and referrals for guests regarding housing options, leasing, and landlord/tenant affairs.

  • Maintains statistics and information needed for reports.

  • Maintains relationships and communication with outside service providers regarding case plans for our guest.

  • Conducts home visits and community visits to former guests of Sarah’s House to provide wrap around

    services.

  • Participates in strategic planning for the program and serves on committees as requested by supervisor.

  • Assists in the identification of programs that will meet the needs of the guests. Assist in development or

    implementation of such programs when requested by supervisor.

  • Maintains records, collect and input data and report statistical information to Case Management Supervisor

    and Program Director.

  • Performs other duties as assigned by supervisor.


This position will require the following:


  • Bachelor’s degree in Human Services, Social Work, or related field and minimum of 2 year related

    experience or an equivalent combination of education and experience

  • Must have ability to communicate effectively in oral and written form; organize and prioritize work; develop

    beneficial working relationships service providers; pay attention to details; work independently and as a

    productive member of a team; and meets established deadlines and goals

  • Experience in working with individuals and families experiencing homelessness preferred.

  • Requires a valid driver’s license with no more than 3 points is required

  • Must be able to work flexible schedules (i.e. nights, weekends, and holidays)

  • This position requires basic casework with minimal supervision and works independently 80% of the time.

Schedule:


  • Monday - Friday

  • 9am - 5pm

*Must be able to work one evening shift per week (12 pm - 8 pm).
