KMHS is Kitsap’s nonprofit, designated community mental health provider for adults with acute and/or chronic serious mental illnesses, and children with serious emotional disturbances and their families. During 2016, KMHS provided services to more than 6,800 children and adults. Medication Services Care Coordinator 
 

As a Medication Services Care Coordinator, you will provide coordination of care for clients of all ages who are assigned to medication management services. The treatment needs for these clients primarily involve medication management, and clients are generally not enrolled in other therapeutic services. This position is a full-time position and requires working with both youth and adult outpatient staff to effectively coordinate care and address needs as they arise.

WHAT YOU’LL DO


  • Provide care coordination services to clients receiving medication management services, including monitoring and updating treatment plans, regular phone contact to assure continuity of care, and effective communication between all involved parties.

  • Routinely monitor and evaluate client stability and treatment needs, referring clients for higher-level services when necessary.  Provide crisis intervention to clients as needed.

  • Communicate consistently and effectively as needed to Medical Providers and Medical Assistants regarding client’s care and concerns

  • Maintain a fluid caseload by connecting clients to community-based services when appropriate, making room for others to be enrolled.

  • Develop relationships with community partners, particularly primary care providers, to facilitate and maintain inter-agency collaboration.

  • Document all clinical services thoroughly and accurately, and submit all documentation in a timely manner

  • Serves as the backup Liaison to Harrison Health Partners staff regarding KMHS clients.

  • Pursue professional growth and development by seeking out learning opportunities and integrating new learning into daily work practices.  Maintain a working knowledge of current trends in community mental health, including diagnosis and treatment modalities, treatment of co-occurring disorders, developments in psychotropic medications, and automation of job tasks using computer technology.

WHAT YOU’LL BRING


  • Bachelor’s degree required. Psychology, social services, or a related field preferred.

  • Minimum of one-year experience working with clients with mental illness required. 

  • Prefer experience in co-occurring disorders treatment, vocational services, or substance abuse treatment.  

  • Possess and maintain a valid driver’s license with an acceptable motor vehicle report and reliable, insured transportation.
