Job Summary



Reviews each discharged patient?s record for incomplete or missing items, identifies the responsible practitioner for each of these items, and links the deficiencies to the appropriate physician for completion to enable the timely completion of the medical record. Verifies chart completion and monitors the reanalysis queue as well as the decline and dictation queue to ensure all records are completed appropriately. Analyzes inpatient, ambulatory, observation, emergency and partial hospitalization program records in accordance with JC standards, MCH medical staff rules and regulations, and other regulatory agencies. Abstracts patient records for statistical data collection using the appropriate computer system.



Minimum Job Requirements




  • Ability to type 30 words per minute.



  • High school education.



  • Previous experience in Health Information Management or general office.


Essential Duties and Responsibilities




  • Analyzes and abstracts all patient discharge records in the EPF and ensures that by the 4th day of discharge 98% of such records have been processed correctly; abstracts all necessary information from the patient records at a 97% accuracy rate and ensures that all records have no missing items and that the physician names correspond correctly to those on physician rosters and physician rotating schedules.



  • Complies with established analysis and abstracting productivity standards as outlined in department policies and procedures and seeks supervisor assistance when such standards cannot be met.



  • Ensures accuracy of discharge codes after researching the death charts.



  • Maintains and submits to supervisor on a daily basis required productivity logs for all assigned tasks.



  • Maintains physician rosters and physician rotating schedules, and updates medical staff signatures as needed to enable the correct analysis of patient records.



  • Processes all duplicate medical record notifications and patient name corrections.



  • Responds to physicians' questions related to their deficiencies and how to access assignments to complete their deficiencies. Directs physicians to appropriate person for clarification as needed.



  • Runs the "Demand Inpatient/Outpatient Incomplete Abstracts by Discharge Date" report daily, identifies records with incomplete abstracts, and completes abstracts.


Knowledge/Skills/Abilities




  • Ability to communicate effectively in English both verbally and in writing.



  • Basic knowledge in use of computers and printers, and/or ability to learn appropriate software application(s).



  • Knowledge of medical terminology.



  • Must be able to maintain confidentiality of sensitive information.



  • Must be able to relate cooperatively and constructively with customers and co-workers.



  • Must be proficient in alphabetical and numerical filing systems.



  • Requires good organizational skills and must be adaptable to frequent changes in assignments.
