To deliver discharge planning and social work services during normal business hours and the entire hospital after hours. Requires interaction with a variety of patient populations including, but not limited to, neonatal, pediatric, adolescent, adult and geriatric. Provides direct intervention with patients and families and jointly with the health care team, provides assessment, problem solving, advocacy, community referrals and crisis intervention services. Responsible for the daily management of all complex discharge planning and social work functions.

Experience REQUIRED:
Minimum of one (1) year of clinical social work experience
Experience PREFERRED
Clinical social work experience in an acute care setting
Related experience in acute discharge planning, home health, case management, long term care or related community agency
Education/training REQUIRED: (Note: work experience may be considered in lieu of credentials not required by law with HR approval. Work experience must be specific to role)
Masters of Social Work Degree from a Council on Social Work Education accredited program
