    The chief objective of this position:

    
    
    
    
    • 
      To deliver discharge planning and social work services during normal business hours and the entire hospital after hours. Requires interaction with a variety of patient populations including, but not limited to, neonatal, pediatric, adolescent, adult and geriatric. Provides direct intervention with patients and families and jointly with the health care team, provides assessment, problem solving, advocacy, community referrals and crisis intervention services.       Clinical social work experience in an acute care setting
      
    
    
    • 
      Related experience in acute discharge planning, home health, case management, long term care or related community agency
