The Intake Coordinator will serve Physicians Choice Home Health Care by communicating with referral sources, physicians, third party providers, agency peers, and clients to facilitate a thorough and complete admission process.

Major Responsibilities:

Receive referrals, physician orders, and all clinical data from referral source. Communicate this information to nursing Team Leaders or appropriate clinical personnel in a timely manner and provide written documentation.

Confirm insurance coverage, benefits, authorization and/or predetermination for care to be provided, as needed.

Maintain accuracy in obtaining and recording appropriate patient, insurance, physician, and other information.

Support ongoing Quality Improvement activities to ensure quality patient care and adherence to Federal, State, and company standards and regulations of operation.

Education/Experience Requirements

Current, unrestricted license as a Registered Nurse or Licensed Practical Nurse in the State of Nebraska or meeting the requirements to practice in Nebraska through the Nursing Licensure Compact.

Experience in the home health care industry is a plus.

Basic knowledge of health insurance, Medicare, Medicaid, and managed care reimbursement guidelines and regulations as they relate to home health care.

Excellent communication skills.

Excellent organizational skills and attention to detail.

Ability to perform accurate and efficient data entry in a timely manner.

Physical Requirements

While performing the duties of the job, the employee is regularly required to sit, talk, and hear. The employee is frequently required to use hands and arms. The employee is occasionally required to stand and walk. The employee must occasionally lift up to 20-50 pounds. The noise level in the work environment is usually moderate. The type of equipment used in this position consists of personal computer, keyboard, and basic office equipment.
