Responsibilities

A day in the life of an Admissions & Social Worker, at Hackensack Meridian Health includes:

 

    Responsible for admissions to facility in a caring, professional and efficient manner and the registration of admissions into COMET
  • Coordinates admissions with facility team and ensures approval by nursing and coordinate with other discipline

  • Census meetings, updating HMH shared report

  • Responsible for the accurate collection and recording of pertinent information

  • Responsible for the greeting of patients/residents/families and tours

  • Monitor bed census

  • Track and work with Referral Sources

  • Work on Marketing Plan to increase census

  • Notify the Board of Social Services of admissions/discharges

  • As part of Social Services team, provide advocacy, education and support for patients and families

  • Evaluate emotional needs and documentation of assessments, progress notes, discharge planning, MDS, Care plans, Advanced Directives and discussing end of life issues

  • Liaison between family/ Resident and staff, facilitating IDC meetings

  • Ensures customer satisfaction by addressing complaints, concerns, suggestions, and providing appropriate follow through.

  • Communicates with community and internal departments regarding financial status of residents, maintain continual insurance reviews and authorizations, referrals for Medicaid coverage

Qualifications

Education, Knowledge, Skills and Abilities Required:


  • CSW, Current NJ Licensure

  • Long Term Care experience required

  • Bachelor's degree is preferred

  • Strong communication and analytic skills are required
