POSITION SUMMARY:

Provides quality psychosocial care to our patient's and coordinate care with medical team. Through the coordination of care, the primary purpose is to conduct a thorough assessment of patients' needs, understanding of the medical plan of care, coping abilities, and barriers to care then implement interventions and/or provide resources. Serves as an integral member of the health care team delivering psychosocial care in accordance with JHACH policies and procedures and state and federal regulations.

LOCATION: St. Petersburg, FL

HOURS: Per Diem

QUALIFICATIONS:

• Master's Degree in Social Work


• Completion of JHACH's intern program to meet the qualifications of the Social Worker career requirements OR successfully completed a comparable intern program in an external healthcare environment


• Excellent listening and oral/written communication skills


• Ability to document effectively in the electronic medical record
