Bread of Life Ministries (BOLM) is in search of a Comprehensive Case Manager to coordinate care for homeless or at risk of being homeless individuals and families requiring assistance from social service agencies. The ideal candidate will work both independently and collaboratively as part of the case management team. These services will assist people to change their lives and take personal responsibility towards obtaining and maintaining a healthy lifestyle. Responsibilities include, but are not limited to, ongoing assessment, developing of a plan of care, utilizing community resources, and referring to service providers. This position works primarily in an office at our shelter, but may sometimes perform local travel in the community. Work experience in social work, psychology, mental health and human services, behavioral health or sciences, human development, counseling, sociology, education or a closely related field is required. Two years of relevant experience working with the homeless and diverse populations is preferred; however, BOLM is willing to develop the skills of the most suitable candidate.
**Requirements** Posses an associates degree, MHRT-C or equivalent training or experience as a case manager. Must successfully pass a criminal background check and motor vehicle record check.
