The Access Primary Care Behavioral Health team is well established with over 15 years of robust integration within the medical home. We work within the Primary Care Behavioral Health (PCBH) model as Behavioral Health Consultants to the primary care providers. BHCs provide targeted assessment and intervention to improve the health and functioning of a culturally diverse patient population across the lifespan. Applicants should have experience with or be highly interested in working with diverse and at-risk patient populations. Applicants should also be interested in working within a highly collaborative team environment.
