Responsibilities


Delivers high quality speech-language pathology services to patients with speech, language, voice, fluency and swallowing disorders, including screening, assessment, treatment, program planning and implementation. Improves patients’ communication skills through vocal exercises and cognitive therapies. May select alternative communication systems and teach their use. Promotes rehab services within the Community, attends meetings and care conferences, performs administrative and billing duties within specified guidelines and timeframes. Demonstrates personal and professional work behaviors which promote the practice of speech-language pathology within the Community. Adheres to applicable principles and practices of speech-language pathology, Brookdale policy and procedures and State / Federal regulations.




  1. Optimizes communication, cognitive and swallowing skills of patients by performing the following tasks: screening, initiating referrals; evaluating and treating appropriate patients; effectively participating in the care planning process; appropriately documenting progress toward identified objectives and communicating patient status and needs to the patient, the patient’s family, the staff and other professionals as appropriate.

  2. Conducts evaluations using standardized tests, specifically selected to assess the individual patient after thorough review of the resident’s medical record, in order to determine patients’ baseline status and identify impairments, functional deficits and potential to improve based on normative data.

  3. Establishes individual patient plan of care, based on current evidence, to optimally address patient-specific goals and to comply with service delivery parameters in accordance with the Resident Assessment Instrument (RAI) Manual, Managed Care Contract or other guidelines specific to skilled nursing and rehabilitation.

  4. Develops treatment plans by identifying a problem list, including long and short-term goals and methods to achieve identified goals. Confers formally and informally with the physician and other team members in coordinating the total rehabilitation program of the patient.

  5. Coordinates treatment plan with patient, family and other team members. Implements and conducts skilled treatment as outlined in the treatment plan. Monitors patient’s response to intervention and modifies treatment as indicated to attain goals in accordance with physician’s orders. Adapts treatment to meet the individual needs of the patient.

  6. Recommends and obtains appropriate equipment to improve patient’s communication. Instructs patient, family and staff in use of augmentative communication devices.

  7. Instructs patient, family and staff in appropriate speech language procedures or home program to allow carryover of therapeutic remediation and goals in daily activities.

  8. Participates in transition / discharge planning.

  9. Reviews the quality and appropriateness of the total services delivered and of individual speech language pathology programs for effectiveness and efficiency, using predetermined criteria.

  10. Utilizes the Community electronic medical record to review the patient’s medical chart, complete forms such as patient screens, physician orders, transition documentation. Documents evaluation, treatment encounter notes, progress reports, re-certifications, discharge notes and other required patient data in the Therapy electronic medical record. Utilizes guidelines for skilled documentation

  11. Submits billing, payroll and expense documentation in an accurate and timely manner meeting all regulatory guidelines. Assists in the collection of statistics as directed.

  12. Adheres to Brookdale policies and procedures. Complies with all Infection Control, Universal Precautions, and OSHA standards for the healthcare professional. Assists with and participates in Quality Assurance and Performance Improvement Programs as required. Adheres to established confidentiality standards as required by Brookdale concerning patient information.

  13. Attends in-service training, as well as, provides education to members of the patient care team and members of the community.

  14. Consistently demonstrates sound judgement in the evaluation, planning, implementation and follow-up of patient therapy programs.

  15. Demonstrates strong interpersonal and communication skills.

  16. Assumes responsibility for ongoing continuing education and professional development.

  17. Complies with Human Resource policies including, but not limited to, maintaining professional licensure in good standing, staying current for personnel file items (TB Test, CPR Certification, etc.), complying with State / Federal / Brookdale wage and hour law regulations, device management and Learning Management System (LMS) completion.

Qualifications


Education and Experience
Master’s or Doctorate Degree in Speech Language Pathology from an accredited program required. Previous speech language pathology experience preferred.



Certifications, Licenses, and Other Special Requirements
Current license to practice speech language pathology within the state. Certificate of Clinical Competency or eligibility for Clinical Fellowship Year.



Management/Decision Making
Uses limited independent judgment to make decisions based on precedents and established guidelines. Solves problems using standard procedures and precedents. Knows when to refer issues to supervisor and when to handle them personally.



Knowledge and Skills
Has a working knowledge of a skill or discipline that requires basic analytic ability. Has an overall understanding of the work environment and process. Has a working knowledge of the organization. Basic computer software. Excellent verbal and written communication skills required. Ability to work holidays and weekends as operation demands. Ability to maintain the highest standards of ethical professional and clinical judgment. Ability to address staff and patients in a courteous and friendly manner as well as accept constructive criticism. Must be willing to communicate with patients of various levels of functioning and various types of personalities. Willingness to communicate perceived physical and emotional needs of patients to other skilled staff to assist with addressing their care needs.
